Hanna Culbertson offers short term personal and crisis counseling for currently enrolled students, as well as referrals to community resources for long term supports. She can assist with common student and personal concerns including but not limited to: anxiety, depression, grief, trauma, interpersonal concerns, stress management, transitioning to college etc.

To make a virtual Zoom appointment with the Wellness Counselor:
- Sign into Advisor Trac with your 800 # and password (same as banner).
- Select "Search availabilities", then "Wellness Counseling". You will see a list of available appointment times in green. Select the one that works for you and then once you have entered the required information click "submit." You will get a confirmation sent to your student email.
- Within 1 business day, you will receive directions in your student email and a link to enter a Zoom virtual meeting with the Counselor at the time of the set appointment.
Alternatively, you can call 541-440-7900 and front office staff can help you make an appointment. Within 1 business day, you will receive directions in your student email, along with a link to enter a zoom virtual meeting with the Counselor, and the time of the set appointment.
Download and fill out the Informed Consent and Intake paperwork below before your Zoom session. Once you have filled out these forms you can submit them as JPG or PDF in the digital forms drop box below. Alternatively, you can print out, fill out and return them in the student center office in person.

The Student Assistance Plan through IBH gives UCC students access to 6 free sessions of counseling with a licensed provider in their state of residence. It also gives them access to work-life balance services such as assistance with legal forms and consultation, financial planning and referral to resources for child and elder care.
