Using this form will upload documents directly to the Human Resources Office. You have three options. Choose one.

1. Upload a photo of your vaccination record here.
2. Download the Medical Exemption Form, print it, fill it out, take a picture of it and upload it here.
3. Download the Religious Exemption Form, print it, fill it out, take a picture of it and upload it here.

The drop box will only accept documents in JPG format.
