Report a Student Conduct Violation
Non-academic violations are behaviors that include but are not limited to, disruptive classroom behavior, alcohol, disorderly conduct, drugs, hazing, harassment, physical violence, student-to-student issues or other violations of college policies, regulations, or rules.
Report an Academic Integrity Violation
Academic violations are behaviors that include but are not limited to, cheating, plagiarism, or falsifying official records related to the College. For more examples please refer to Administrative Procedure #5506.
Complaint / Grievance
Please use this form for any complaint/grievance including but not limited to, facilities, interactions with College employees or faculty members, course grades, administrative procedures, complaints related to academic services, violations of board policies, etc.
Student of Concern Form (CARES Team)
Please use this form if you are concerned about a student’s well-being, know of a student struggling mentally, financially.
Sexual Harassment Under Title IX Reporting Form
This form should be used to report concerns pertaining to possible violations of Umpqua Community College’s Sexual Harassment Under Title IX Policy such as gender-based discrimination, sexual assault, dating violence, domestic violence, stalking, unwelcome conduct based on sex/gender/gender identity, quid pro quo harassment, retaliation, or other forms of sexual misconduct.
