Allied Health Programs Pre-Course Information

2023-2024 Academic Year

Clinical experience is a crucial part of the learning experience and UCC is committed to student success. As such, UCC has partnered with numerous clinical facilities around the county. As guests in these facilities students are expected to follow the policies and procedures as if they were an employee of the site.

This requires Allied Health Students at their own expense to submit/obtain each of the following (please note, all costs will be invoiced to student accounts, except CPR. What this means is students will not have to pay for these things upfront. Those expenses will be placed on student accounts and students will be responsible for payment, however this allows students to use financial aid or scholarships to help pay for these costs).

CPR/Basic Life Support (BLS) for Healthcare Providers

Students are required to have and maintain through their program a current Healthcare Provider CPR Card. The only card that is accepted is shown below (or an RQI card):

Currently scheduled CPR classes:

CWT 10/07/2023 from 9:00 AM – 5:30 PM. Call CWT 541-440-4668 for registration $49.00

HE 260 – 11/04/2023 from 9:00 AM – 5:30 PM – 1 credit class. Contact registration for sign up.

 

Immunization Records

Students must submit documented evidence of vaccine or documented immunity via titer or valid history of disease or a record from ALERT IIS (Oregon ALERT Immunization Information System). Students need to download and complete this form.

Students do not need to complete an account. Email or mail the form to the address shown on the form.

Required:

  • Hepatitis B (Hep B): 3 shot series.
  • Measles, mumps, and rubella (MMR): 2 shot series
  • Tetanus, diphtheria, pertussis (Tdap): required if more than 10 years of last Tdap
  • Varicella (Chicken Pox): date of vaccine or documented date of disease
  • COVID-19

Recommended:   

  • Influenza (seasonal flu)
  • Polio

Students who are not vaccinated against Covid-19 must submit an Oregon Health Authority Medical or Religious exemption. If you plan on submitting an exemption, please email me as soon as possible so I can send you the application. Additionally, students who do not receive a flu vaccine will have to complete a declination form.
